(“Easzo app”) (along with the website and app referred to as “Services”). Easzo provides the services in partnership with its agents, associates, representatives or other third parties (referred as “Partners”)

• Information is collected by the means of registration form during the user using our web portal or our mobile app, the patient will also be sharing his information related to his health conditions and his location, over a video or audio or chat medium. The information will be mainly collected by the means of web application or mobile application and during the call/chat/ video calls with the practitioners / personal word to word during home visits of the practitioner. The information also will be collected during user call/chat with the customer care to cross verify the users concerns regarding the booking and his bank details if for refund.
• The information which is collected by us is both personal information and sensitive personal information as well, All the information provided to us by the User is voluntary. Personal Information under the SPI rules means any information that may be used to identify an individual. This includes but is not limited to: first and last name, email address, mailing and residential address, pin code, telephone number, title, birth date, gender, occupation, company you work for and other information needed to provide a service you have requested. For animals, details such as the breed, gender, age must be listed.
• Whereas, Sensitive personal data or information under the SPI rules relates to: password, financial information about the user, medical record and medical history, physical physiological and mental health condition, diagnostic reports, prescriptions, sexual orientation, insurance data, family details, biometric information, details provided by the user or of the animal at the time of registration, Call data records and any information which is shared by the user to the practitioner
• If any information is made available to the public and is easily accessible by them then such information is not regarded as personal information or sensitive personal data or information under the Rights to Information Act, 2005.

3. USE OF INFORMATION.

• i. The personal and sensitive personal information given by you or for the person whom you are representing or for a pet/animal, any such information collected will be used solely for the purpose of Easzo.
• User personal details like name, phone, address, age etc will be collected for the purpose of providing consultations booking to inform the doctor regarding the genuineness of the users. The symptoms and the duration of the symptoms, his/her health records, scan and health reports will be collected by for the purpose of providing the exact diagnosis of the disease and to provide him/her with the proper medication. Location of the user will be detected only after he/she gives us the permission to locate his address thereby book various at home health care services where the practitioner will have to come home for treatment or for services which will show the nearest available practitioner or the service provider.
• The information provided is also used to improve the services provided by Easzo, this information provided can also be used for non-personally identifiable form for research, statistical analysis and business intelligence purposes. The user hereby accepts through this Privacy policy that Easzo is given the rights to use any content provided by you for communication purpose so as to improve the booking appointments and for obtaining any feedback regarding any practitioners and their practice and for resolving customer service supported related issues. You hereby agree that Easzo can contact you by the details provided, to complete any transaction if it is not completed by you.

• By accessing to the Services then the User is deemed to accept the Terms of Use along with the terms of the Privacy policy. If the user has any disagreement with the service, then the user has the option to discontinue the Services provided by Easzo.
